I noticed my friends account had 210 games in it for the few months, I checked yesterday and he had 209. I asked him and he said he hadn't removed anything.
This means Steam can just..remove a game from your library and you wouldn't even notice. Whether it's a bug or not, point remains.

I've hid 10 games. Mostly demos and shit. Went from 210 games down to 200.
I could of sworn I bought Mad Max but It wasn't in my Steam Library, I bought it in the sale last week. I wouldn't be surprised if it checks your library and removes games you haven't played so you buy them again.

If a game gets delisted (fake games kept getting delisted) it won't count towards your game count anymore, but you can still download the game.
Volvo has removed games from my library, but you do get a warning in that case. Like when there were millions of dirt 3 keys leaked, mine was eventually removed.
