What does it do?
isaac in general has a shitload of bad decisions
Coins have a 10% chance to be replaced with sacks. Bombs, keys, cards, runes, and batteries have a 20% chance.
This includes pickups spawned by most sources: room rewards, chests, slot machines, special shopkeepers, etc.
The exception to this are pickups spawned by Isaac himself directly, e.g. with the likes of IV Bag IV Bag or Fanny Pack Fanny Pack, and contents of other sacks.
Spawns a sack upon pickup.
Basically one of those "lol free resources" items.

It has a small chance each time a coin is spawned to spawn a sack that drops a bunch of resources, including coins.
Yes it can loop indefinitely theoretically.
Replaces the occasional pick-up with a sack. This could make objects and items that drop a very specific pickup also drop sacks. For example, blood banks could drop sacks instead of some coins. Since sacks may contain other pick ups, it could very easily fill up your bombs and keys reservoirs as well as provide you with whatever cards you like.

Who cares? The player is free to use it or not use it however they wish. Do you also complain about optional game breaking exploits in single player games?
There's a lot worse bullshit you could be complaining about in BoI, such as Cursed Eye, Soi Milk, half soul hearts purely to punish frequent resetters, the donation machine randomly breaking so you can't donate to the shop too quickly, the majority of cursed floors making the game more tedious and annoying to play rather than being challenging, HP scaling bossfights, bulletsponge enemies, etc.
Yet you pick one of the new beneficial items for the player.

I prefer my roguelikes to be robustly balanced so that playing at my best doesn't feel cheap. Having to handicap myself to have fun is very lame.

Not him but Isaac has just as bad of an issue with being too generous as it does with anti-fun attempts at adding difficulty like what you listed. Every run is either absurdly slow or you can empty every room in frame one.

I don't think I ever won a game because of "sack head", it just gives easier bombs and keys that can plentiful in a bunch of other ways like everything in Isaac.

Only the starter roaster that is literally to make the player familiar. Want the normal mode? Play as Isaac or Lazarus, want easy mode? Play as Azazel, Samson, or have more health with Madelaine? Want to get into skill? Play as Eve or Judas.
And this is just the starter roaster, later you get restrictive harder characters like Blue Baby, Lost and Keeper, some other gimmick characters like Abbadon and playstyle changers like Lilith and Forgotten. Each character has key itens to get in a run that will make it easier or harder. It isn't the same.

Mom's knife is trash synergy killer, any item that change your tear for another thing (brimstone, the tech itens, mom's knife) is a synergy killer. Also, overpowered itens is part of a roguelike.
