All they have to do is make it more like Terraria, and you'll have a game of the decade on your hands, instead of something that's simply okay.

Tons of games do this just fine without needing to pause combat. Another example would be kingdom hearts, which not only restricts your options when in battle, but you have to find a safe spot and a safe time to heal inbetween enemy attacks. Some don't even give you that window. I was thinking of just removing the pause that happens whenever Link accesses his items and weapons, make the player work while on the go.

Both. The menu should only pause the game if there are no enemies nearby, and if you need the game to be paused, have a dedicated pause button that stops everything.
I just think that a sprawling adventure should always leave you looking behind your back. Safety should be a commodity granted by towns, not just by the pause of a button.
Nah, no slow down quick menu would be cancerous vs the number of weapons you carry. It'd prevent combing and taking advantage of different weapon properties. Make the games already rather basic combat system even more dull.
