Kissless virgin who feels invisible starts to actually disappear and has to figure out why. Gameplay is like a reverse stealth game where you have to stay within the line of sight of people and you control the camera by jumping into a first person view but you still control the character. The Zeno Clash devs tried and it was fucking terrible.
Otherwise I'd say that rice girl game is the next closest thing.

Realistic medical system tied into some sort of gore mechanic.

Like something that'd keep track of your blood, oxygen levels, major organs, ect, all the medical items do things realistic as far as like bandages, torniquets, ect, and also tied directly to gore.

Someone shoots a head with a 12 gauge, it destroys realistically, but also for legs, arms, but tied all in so you don't die from a blown off leg until the blood loss catches up with you.

So any zombie survival?

Lets see.
1) Realistic medical system. Almost none of them have anything advanced, half of them just use a very basic HP system.

I mean like, shoot a lung, your stamina's affected as blood fills the lungs. Leads to coughing, and the bleed also has you fade out. To fix, you actually need to treat for pneumothorax, so bandage allowing it to pass, and a needle or something to let the air out to allow the lung to refill.

Broken leg-forced prone, forced crawl. Must have way to reset bone, apply splint and need to hobble for time to heal.

Eating a poison requires a particular antidote, and effects you based on what it is, nerve toxin, cytotoxin, hemotoxin, ect, and the body reacts and stuff, reflects blood pressure and heart rate, ect.

And tied into that, how many actually have real sort of gore.

Not like Soldier of fortune where you click a head and the head just disappears with weird generic gibs, but like, skull peels back like broken egg shell with tape, brains splatter, and the person's face is stretched and distorted as appropriate.

Bullet hits spine, paralyzed but not dead because your brain is still intact.

Bullet passes through you and will track what organs it does or doesn't hit and gives you the appropriate medical response. Not broken down into super basic like torso/head, but specifically having Brain, eyes, spinal cord, heart, lungs, liver, kidneys, spleen, intestines, major blood vessels, mapped out with their own hitboxes and give effects based on each of those, and not just head x 2 multiplier, if it blows off your jaw, that's not your brain, so you're hurting but your not dead, but now have to worry about blood loss (and if there's VOIP with it, lose ability to speak, ect.)
