Considering every time someone tries to make a change they get kidnapped killed and left in some dump to be discovered a month later and nothing gets done, I'll go ahead and say they're quite influential.

In rural towns and small cities the cartel manages every aspect of daily living and everyone knows who's doing what and who's working for who.
In bigger cities they may have regional influence as long as they support/cuck the governor/major. They may not be able to control every aspect but they sure as hell have a saying on some public policies, specially in law enforcement.
The "government" sure as hell knows who's crooked and who is not, and this is something I concluded based on the access to smartphones and the privacy scandals related to domestic espionage. It's innocent (idiotic, actually) to think the State is totally ignorant on the amount/location of narcos. Specially since those fuckers are addicted to their social media and wear their phones as jewelry.

Maybe calling them a narco state is far fetched, but they most deffinetly have a legit underworld narco industry, not big enough to be in the same category as the big leagues like México, Colombia or the russian mob but sure enough the biggest in western europe
