Well, not exactly incomprehensible because unless they go all out with the slang they are comprehensible for native speakers, but Spanish learners that didn't focus on a Caribbean dialect claim that they had the hardest time understanding the Dominican dialect of Spanish (it shares said fame with Cuban and Chilean Spanish). I'd imagine that's a mix of the fact that they do a lot of contractions (even more so than other Spanish speakers), aspirate the s, speak very fast, etc. However I'm not an expert on Caribbean dialects of Spanish and I'm sure someone else could give you a better rundown.
