Health insurance covers some percentage of the costs and if you make below a certain amount of money you can get Medicaid to cover it. Otherwise, you can just get billed and never pay it.

That’s true but if they’re that poor they’re also eligible for Medicaid, which I believe covers most if not all pregnancy related costs. The effect on prices is the same either way.

You fail to pay the bill, the hospital sells the right to collect the debt to a sleazy collector for 10% of the full amount. The collector then takes all the debt they bought and hounds everyone threatening to sue for years until a timer expires and they legally can't collect anymore. They'll still try to, but you don't have to talk to them.

If they think you actually do have a least some money they will sue you for it though. There's also the issue of recurring treatments, where you can't just not pay because you have to go back to the doctor the next week. Those are the situations where people wind up taking out loans/declare bankruptcy.
