A lot of cities tend to have 'old towns' somewhere in the centre.
I struggle to think of a town here that doesn't have at least one or two medieval landmarks left.
In central Italy and the North mostly. There is always a whole lot of LAYERS around the medieval stuff though, you may not be able to spot it. Pic related, for example.
I reckon Southern France is similar.

Pretty much everywhere. Italy is great, the German state of Rhineland-Palatinate has tons of neat castles on the shores of the Rhine. Prague is the largest old town center, tons of cities have cathedrals. Is there a region you always wanted to visit? Chances are you can see a lot of medieval stuff near that.
Almost anywhere, but not in the quantity you may be looking for.
Visit any non bombarded in WW2 large European city and go to the center, it doesnt even have to be big city, but in smaller cities there is naturally less to see.
All the cities are very similar in the region, for example Austrian and Czech cities will look similar, so pick your preffered aesthethic
Germany. Check out Sachsen-Anhalt, the Rhineland-Palatinate, Rhineland part of Rhineland-Westphalia, and Franconia.
Koln has some Roman ruins as well as the greatest Cathedral ever built

I really liked it. City felt really "authentic" or something like that. Don't know why every German I talk to hates the city, although I also loved Berlin so maybe there's just something wrong with me
Aachen (Rhineland-Westphalia) felt medieval too. Still has that old town square and everything even if you ignore the Cathedral

If you want city walls then come here to Visby, it's from the 11th century and still standing.

Most of Germany was destroyed in WWII so most of the buildings are rebuilt modern copycats that don't look as nice as the originals. Compare Germany/Austria and Switzerland, where it wasn't really bombed, and you can see a huge difference. I'm sure there are still some pockets that were saved, but the majority of those cities were rebuilt.
Switzerland, France, and Italy all have very old, beautiful towns everywhere, and they are authentic, not modern reproductions. Literally every little town of over 5k I went to in Switzerland had an old town that was jaw dropping. I haven't explored Italy and France as much, but based on street view, they're similar.
