Being ugly is a choice for women. She can get contacts, make her hair, do her makeup. Notice Im not even saying to lose weight or anything serious. Just minor cosmetic effort.
If they chose to be ugly then they have some sort of mental issue like autism or something. People just ignore them because they are obviously special needs and to be pitied.
