Definitely region. I'm still an American but I'm a New Englander first. The US is the size of a continent, just because we all live under the same federal government and share a language doesn't mean we're a cultural monolith. New England and the canadian maritimes are more similar than new england and arizona or the maritimes and alberta. Same goes for ontario and the northern midwest and for washington/oregon and british columbia. When it comes to culture in gigantic countries, regions are much more important than nation-states.
