Telling people that a good diet and regular exercise can make them look like steroid users probably turns a lot of people off from exercising when they don't get the same results and get bummed out.
Guy in the OP doesn't really look that huge though, but to look like that you have to do a bunch of unhealthy things like dehydrating yourself to make your muscles stand out more.

I don’t think I’ve ever beat a truly hard game but I have done truly hard tasks in otherwise easy games
The hardest games I’ve beaten are probably Sekiro or Super Meat Boy though
Maybe MGRR Jetstream Sam VR04 on Revengeance difficulty with no sub weapons, no upgrades, and no healing items.
I have a hard time thinking of anything that took more work to learn how to do than that.
