In the end Samuel challenges all of his fears, reconnects america, finds will and reason to live, helps his father to move on from hell and goes off the grid to raise Louise.

You have to travel across Iceland delivering essential/non-essential items to NPCs in order to help stabilize the country and establish what's known as a Chiral Network at the behest of the government. But it turns out that doing so plays into the heads of an entity that will bring about the destruction of mankind through said network. In the end, said entity doesn't really want that and just locks itself away for all eternity after the main character hugs it.

In the years 2020 homo demons promoted sexual deviances that led to decline of birth rates (see email Asexual World). By using their blueless rainbow they established a link to the world of the dead (see email No Blue in the Inverted Rainbow). Propagation of abortion across the world started to break the wall between the world of living and the dead. Living women had dead babies inside them. Kojima dances around the issue of abortion in the game and focuses on stillborn babies but it's clear what the main cause of dead babies linked to living mothers is.
Eventually Jesus Christ says "I had enough" and breaks the wall between two worlds entirely, a woman with a dead baby inside her becomes a door between two worlds and causes a phenomenon known as Death Stranding, a chain reaction of big bangs caused by Jesus to punish people for their sins.
In order to repent for the sins of the human race Sam needs to go around America while being metaphorically pregnant with a baby girl. In the end of the game he metaphorically gives birth to this baby girl and breaks the cycle of abortion and death. Jesus saw the baby and he saw that it was good. And mankind was forgiven.

The apocalypse began when the dead became trapped in limbo until they later gain an entry way back into the world of the living which starts distorting the laws of reality, alters the world, and chemically changes rain with the "chiral energy" trapped in the atmosphere.
The apocalypse occurs wiping out a majority of the population due to many factors caused by these events, among them large explosions like atomic bombs caused by a chain reaction of people dying and violently exploding due to the soul and body reacting with one another as opposites.
Sam was one of the first in a new program using the infants of stillborn mothers as batteries due to their connection with the world of living in dead, until Sam's father eventually attempts to free him breaking him out of the containment tainting him to become unusable in the experiment but still having the connection with the world of the undead. Sam is unaware of this circumstance and is told that he is the child of the president but has a unique ability to sense the dead around him so he is one of the few people who can effectively carry supplies from point to point.
Once he reaches the president on her deathbed, she request that he saves his "sister" and restores the Chiral network to all the cities across the "United States" (basically an internet storing endless amounts of data in limbo), so he goes about doing it restoring communication until eventually, he reaches the West Coast connecting restoring the chiral network to the largest extent he can. What Sam doesn't know, is that the President (his sister) was actually a god-like creature who was tasked by an unknown force to be the finality of life and the network gave her the power to do as she needed to.

Due to Sam's bond with her over his lifetime as both a child and a brother, he convinces her that humanity is deserving of more time for how much effort they've put in despite all circumstances being against them so she instead chooses to isolate herself completely and break all connections from everyone and everything to live alone for all eternity.
Sam returns from her limbo back into the world of the living, but the infant he was given as a tool begins to pass away and Sam is told he has to incinerate to avoid an explosion, but if he's willing, he can take it out of it's enclosure and see if that saves it even though the odds are hopeless. Sam reaches the incinerator and takes it out with no response and begins to have visions that he thinks is the infants, but is really his own when he was enclosed and his father rescued him. After a few moments, the infant starts to breathe and Sam instead incinerates the tracker ending his career as a delivery man in most likely living somewhere comfortably with B.B. off the grid.
Obviously, I took some liberties in explaining this without the phrasing in the game because I didn't want to explain all elements and why they're called what it's called, but it's a pretty straight forward plot. I find it reasonable for the characters to be so bizarre in how they act because everyone's been trapped in bunker like cities or just alone all together in a world on it's last leg that no one has a great understanding of what will happen next or why things are the way they are.
I get how this game could turn people off, but I enjoyed the experience so much for how unique it was and how much fun it was putting all the pieces together.
Anytime someone dies they blow up. One dude can come back after dying and his wife/sister/mentor asks him to hook up the entire US with DSL. He does this. There's also a baby in a jar.
