In Shantae games, you can transform into variety of creatures using dancing. Includes harpies that can flap through the air, mermaids who swim in water freely, and so on. Not everyone's cup of tea as games tend to be pretty simple otherwise. Also fanservice since it's now a problem on gay/v/.
In Megaman ZX and it's sequel, you have forms you can freely choose. In first game, all forms are human-like, but differ in abilities and weapons. In sequel, you also can transform into replicas of bosses you have defeated. They are often impractical, but still.
In Megaman X7 and X8 you can play as Axl, who can transform into some of robots he defeats. Not very useful, but still it's there. Also X7 is not a game I'd advise to play anyway.
In The Legend of Zelda: Majora's Mask, you can collect masks that transform you into various beings, who posses different powers.
I haven't played it myself, but I think Altered Beast has transformations into various monsters too.
There was also some other game where you transform each time you defeat the boss, and supposedly become demon in the end while saving your daughter, but I can't recall what game was called.

In Ecco the Dolphin there are sections where you are transformed into alien thing (I think. Check with someone else, I may be mixing things up).

Can't remember the name but there was one old game about some guy who gets stranded on an island controlled by these cultists turning people into insects with some royal jelly. I remember there being a magic system and at multiple parts throughout the story you would eat some of the royal jelly, slightly transforming you into one of the insects more and more each time you ate it. There was also this spider that controlled time and had put you in some type of time loop. It was top down if that helps.
