Breast size increases your charisma but decresses agility and stamina.

The A Dance With Rogues module for Neverwinter Nights has a very straightforward system. The PC's bust size depends on the CON stat, which is pretty useless for rogues, as you won't be facetanking.
There are also fairly restrictive minimum stats, specially charisma, which would otherwise be the main dump stat. So if you want larger tits, those CON points have to come from somewhere, making the PC dumber or weaker.
Unfortunately, boob scaling only goes up to 14 CON, which isn't that high, costing only 5 points. 18, the max, costs a whoping 17 points, which would require massive gimping of the other stats.
Would be amazing to play as a princess turned rogue, who simply sucks at it because she has to carry several kilos of boob meat

You choose bust size at the beginning of a mission along with other physical measurements/stats as a loadout. Increasing your bust size increases your total mana, but also makes enemies more aggressive as they increasingly want to capture you to sell to a mana farm.
It's a natural consequence of minmaxing for a low-priority stat
And it's specifically not attractivness, that's off CHA. The PC in that module is canonically at least fairly pretty, so minimum CHA is 14

ironic because i've always considered the two things going hand in hand. a bigger bust translates in the woman having high confidence in herself and not the other way around

That's not always true
well, i had to streamline my thoughts for thinking in rpg-terms. i have to admit that throwing another stat into mix will certainly open up a lotta more possibilites for your game and honestly, it's probably the better way of thinking given what i was basing some of previous ideas on
for example, i thought wardrobe malfunction as a legit but random mechanic during battles with the gal fixing herself up (and therefore losing her turn in-battle as a result) in case it ever happens. but that would kinda go against my original thought of "well, she has big tiddies therefore her confidence is certainly very high", wouldn't it?
At least until the very end when they don't want to come back out.
