I can't remember being scared of anything from video games because I had ultimate mortal kombat 3 for SNES when I was in preschool and somehow figured out to use 2 controllers to get through tournament mode and select a prize at the end which led to seeing the supreme demonstration of all of the fucked up fatalities. I thought it was cool and funny and showed my mom and she ended up throwing the game away, though.
