Balance issues really only exist in video games, actual games tend to be extremely black and white. The only balanced and somewhat complex multiplayer game I can think of is pinball, but even that's just the single player experience with a score comparison.
Remove random crits and I'd argue Team Fortress 2 vanilla is balanced.

Turn based games like chess can't be considered perfectly balanced, not that it makes a noticeable difference but if we weren't getting autistic about it we wouldn't be discussing it on a Polynesian snipe-hunting forum.
Videogames are nigh impossible to balance. I think some of the game modes of the older Halo games get close in terms of shooters.
Recently I think the only one that even closely comes to mind is some of the racing games like the CTR remake.

Maybe I'm wrong, but I remember 9v9 TF2 (As in, one of each class) being more or less pretty balanced in that everything had it's job and did it well. The only reason why 6v6 was THE thing is because Soldiers and Demos are so fucking overpowered that everyone decided it's "more fun" to just build the game around them at that point because everyone loves them and how impactful they are, and because Scouts are practically the only thing that can reliably avoid splash damage. It's been a long long while though, so I'm probably wrong, that's just what I remember though. Fucking Soldiers and Demos are so boring though, man.

When two players play perfectly black can't win and at best can draw, how is this perfectly balanced?
Some maps are heavily skewed towards either team, some weapons are still straight upgrades, and other ones are so useless that they're not even worth using as a joke. Pyro is fundamentally bad, sniper does spy's job better than him, soldier/demo/scout are insanely strong when played correctly, and medic is insanely overpowered.

Which is only the meta because Soldiers and Demos are leagues above everything else in power and utility though, right? Like, they're just flat out the best classes in the game.
In which case I guess TF2 doesn't qualify, because it's meta does not involve regular use of all classes or characters, so I would be way wrong. So I've got absolutely nothing then, I can't think of a single balanced multiplayer game. It's likely an impossibility.
