The primary hatch? That isn't possible. Protocol for anything short of a crash dive is strictly computerized these days, someone in ops would've had to override the safety controls. I spent six years on an Ohio. Nobody is inept enough to design a sub without computerized safeties except the Russians.

It probably wasn't their fault. The Pajeets rented an Akula off the Russians back in the late 90s and tried to copy it. Russian subs have notoriously bad safeties because they drill their men to do it all manually instead of relying on electronic checklists, even though they still have them. My guess is some guy was drunk and didn't close the hatch, and because the thing was built on a budget it didn't show up in ops and they thought it was safe to dive. That, or, they were doing crash drive drills and had a mechanical failure on the conning tower hatch.
I can't stress enough how impossible it would be for a fuckup like this to happen in an American vessel short of battle damage or a crash, but the fact that they didn't all die instantly means that at least the bulkheads were secure. The real culprits here are whichever firm they had copy the Akula.
