Domestic healthcare, reforms, etc. are all ultimately secondary because if imperialism isnt primarily addressed those reforms are just going to shore up American society to continue the imperial project and be much worse in the long term. First strike weapons, not complete unilateral disarmament. The goal of U.S. nuclear policy is to be able to threaten a massive counterforce first strike, knocking out all of Russia / China's command and control bunkers and ICBM missile silos in one surprise attack. The credible threat of this allows U.S. imperialism to reserve the right to use tactical nuclear weapons and generally get its way in great power confrontations - like pointing a gun at someone's head, the nuclear arsenal is used, even if it's not fired. Read "A Call to Mutiny" by former nuclear strategist Daniel Ellsberg for a more detailed rundown on how nuclear blackmail is used: ellsberg.net/call-to-mutiny/
The drive for strategic nuclear supremacy is why you see the U.S. again pursuing new first strike weapons. The I.N.F. withdrawal is the major one, reducing the flight time of a first strike and allowing the U.S. to threaten Russia or China separately without flying ICBMs over the others' radar. Or the new initiatives in missile defense - there's no way in hell that missile defense could defend against an enemy first strike, but it could plausibly defend against the "ragged retaliation" left over after a U.S. first strike. All these first strike preparations also increase the chances of an accidental nuclear war. The best way to "survive" a nuclear exchange is to launch a preemptive first strike to destroy their weapons first. When something funny shows up on early warning radar, there's all the more reason to believe it's real, especially when INF range missiles reduce the time possible to make a decision down to 2-3 minutes (including the time to wake the generals up).
The solution here is stop threatening nuclear first use, adopting a No First Use policy. To make this policy credible (the Soviets adopted NFU in the 80s but it was ignored) we'll also have to unilaterally disarm weapons accurate enough to make a first strike: shut down all ICBM and IRBM programs and rely on nuclear submarines and a small fleet of bombers.
What this ultimately means is that without the threat of unleashing thermonuclear armageddon, the U.S. will either have to cede some of its imperialist ambitions (unacceptable to porky) or dramatically increase its military spending even further to deter rivals with conventional force. Generation after generation of cold warriors, Kennan, Kissinger, Schultz, etc. recognize how close they came to nuclear war and actually support this policy, but porky is resistant because he'd rather spend his money on yachts than aircraft carriers. This will expose the true scale and consequences of American imperialism, and how porky has routinely gambled the lives of hundreds of millions of people in pursuit of cheap oil and labor. It's better than letting things continue as normal, only to meet the real, thermonuclear End of History after an error of judgement.

American Disarmament of first strike weapons won’t happen because it isn’t in America’s self interest. This is especially true as American Imperialism falls apart, America will rely more and more on these weapons because It’ll prevent China from doing to them, what they did to the world. And at the end of the day people act in their self interest, and first strike weapons are incredibly useful, to useful to give up because of some moral principle. The only way America would disarm it’s self of first strike weapons is if it was forced to from the outside, and at that point is when they are used.
Because of MAD, America would only use these weapons for real if America first lost it’s position as an Imperial power, second it’s self-determination was threatened, and third was given an ultimatum to give up these weapons.
